Lasers Shine Precision Light on Beautiful Skin

When lasers first became popular in the early 1980s, they were overused without achieving significant improvement. Rapid and significant technological improvements in the last 10 years have truly revolutionized today's plastic surgery procedures and results. Laser skin rejuvenation from New Jersey plastic surgeon Dr. David Evdokimow can improve wrinkles, skin texture, large pores, acne scars, other scars or stretch marks, age spots, rosacea, vascular conditions (port-wine stains and spider veins of the legs and face) and unwanted hair. Today's lasers can treat these concerns without surgery, significant discomfort or downtime.

VISIA skin analysis

The first step in the laser skin rejuvenation process is to identify the problems. To assist in this process, a VISIA digital skin analysis can be performed (at no additional cost). VISIA evaluates the severity of skin conditions such as fine lines and wrinkles, hyperpigmentation, pores, acne, sun damage and variations of color and pigment distribution. VISIA analyses can be repeated throughout the treatment process. Based on the physical exam, medical history and VISIA findings, we develop an individualized treatment plan. We use photographs to help track improvements.

The majority of skin rejuvenation procedures can be performed right in the surgeon’s office. However, if they are combined with other procedures, treatment at a hospital or surgery center may be necessary. We perform most treatments under local anesthesia. There are exceptions; for example, treatments such as IPL require no anesthesia at all, while non-fractional laser resurfacing may require general anesthesia (as it is more invasive).

Depending on the laser used and the goals of the procedure, the target tissue absorbs specific wavelengths of laser light. The tissue is then destroyed or stimulated. More than one treatment may be necessary to achieve optimal results. Generally, the less invasive the treatment is, the greater the number of treatment sessions required.

New advances in laser technology, especially the use of fractional lasers and nonablative skin resurfacing, can improve scars and stretch marks without surgery. Dr. Evdokimow routinely uses fractional, nonablative lasers that affect the deep layers of the skin without interfering with skin surface. In addition, recent advancements in laser technology allow Dr. Evdokimow to use a combination of nonablative and ablative fractional lasers to rejuvenate the face and treat stretch marks with superior results. Lasers stimulate fibroblasts (the collagen-producing cells of the skin) to produce more collagen. They also seal broken blood vessels. The end result is improved color and smoother, healthier-looking skin. 

Scar removal

Whether your scars are from acne, surgery, burn, insect or animal bite or an accident, they can seriously impact your confidence, cause pain and affect quality of life. In severe cases, scars can also limit mobility. Dr. Evdokimow will take into consideration the characteristics of your scar before choosing the most suitable approach. Surgical scar revision is the first line of treatment. To achieve optimal results, Dr. Evdokimow also combines nonsurgical treatments such as:

  • Fat grafts
  • Fractional lasers
  • Nonablative skin resurfacing
  • IPL (intense pulsed light)

These treatment options deliver excellent results with less discomfort and minimal downtime.

Laser scar improvement is appropriate for all skin types. Acne scar treatment can be particularly challenging as there is a large variation in scar appearance, and treatment depends on the type of scar. Often, laser-resurfacing treatments are combined with injectable fillers and surgical excision. 

Stretch marks

Most of us are familiar with stretch marks. They are those parallel lines of overstretched skin. The appearance of the stretch marks varies according to age – initially they are raised and red, eventually becoming a lighter color and depressed. 

Hair removal 

It seems that each conventional method of hair removal is rife with problems. With waxing, there is the potential for allergic reactions, burns, scarring and ingrown hair. Chemical depilatories can cause skin irritation and must be repeated regularly (usually every week). Plucking is painful and can even cause scarring for some people. There is a better way. We employ two different types of hair removal: intense pulsed light (IPL) and laser hair removal. Each uses a different device and accomplishes hair removal in a different way:

IPL HAIR REMOVAL: For IPL hair removal, we use Palomar StarLux®, the most versatile IPL technology. During IPL, a high-energy beam of light is passed through the skin to the hair follicle. When it is absorbed by the follicle, the light energy is transformed into heat. The heat destroys the hair follicle but leaves surrounding skin unaffected. It is appropriate for all skin types as well as all hair colors – and even works for patients with finer hair.
LASER HAIR REMOVAL: During laser hair removal, the laser light is absorbed by the hair pigment, causing damage to the hair follicle without significant damage to the surrounding tissue. This process is known as a selective thermolysis. With the current advances in laser technology, there are lasers available for all skin types. However, in general the best results are achieved in individuals with very light skin and very dark hair. Laser hair reduction is not effective for people with very light and blond hair or gray hair. 

Laser tattoo removal

Everyone makes mistakes. If you feel like your tattoo is one of those mistakes, you’re not alone. In the last 30 to 40 years, the popularity of tattoos has exploded. It’s a full-fledged cultural phenomenon and it is most popular in people between the ages of 18 to 30 who believe that a tattoo will make them more attractive and sexy. Unfortunately, this same group undergoes the fastest, most significant lifestyle changes, and the tattoo that was once sexy often becomes an embarrasing burden.

Lasers have become the most popular method of modern tattoo removal. During laser tattoo removal, laser energy breaks the ink into particles so small that they can be absorbed and removed through the body’s immune system. Q-switched YAG lasers represent the "gold standard," with multiple wavelengths to cover a broad spectrum of colors. Dr. Evdokimow uses the HOYA ConBio Revlite laser with the addition of PhotoAcoustic Technology Pulse for improved results.

Several laser tattoo removal treatments are usually needed. The number of sessions required depends upon the amount or type of dye (or combination of dyes) used, the experience of the tattoo artist (a less experienced tattoo artist may lead to easier removal), the age of the tattoo (newer tattoos are usually easier to remove) and the depth of ink injection. Generally, six to 12 treatments are required with six weeks in between treatments. For many people, however, surgical excision is the most time- and cost-effective method of tattoo removal. Excision achieves the best aesthetic results when the resulting scar can be positioned in a natural skin crease.
